School nurse plays a crucial role in maintaining a healthy and safe learning environment, ensuring that students are physically and emotionally well, and contributing to the overall success of the educational experience. Nurses serve as a resource for students, parents, and staff in addressing health-related concerns and promoting a culture of wellness within the school.

If your child is ill, please keep them home until they are feeling better. Students must be fever-free (<100.4) for 24 hours without fever-reducing medication. Students must also be free of vomiting and diarrhea for 24 hours before returning to school.

Medication Administration

In order for a student to receive medication during school hours (prescription or over the counter), a Permission for Medication form needs to be completed by a physician, signed and stamped by physician’s office, and parent/guardian signature is required. Acetaminophen, Ibuprofen, Tums, and liquid Benadryl are available in school. Any other medications, that a student requires, must be delivered to school in its original container. DO NOT SEND PILLS IN A ZIP-LOCK BAGGIE! Parents cannot give verbal permission to give medications. The form must be returned to nurse’s office via email, fax, drop off in person or mailed in order for students to receive medication.

Asthma

The Middle Township School District must have a health plan in place for students with a health concern. The health care plan helps to ensure a better understanding of your Child’s healthcare needs, and is reviewed with your child’s teachers and principal. Please take Asthma Action Plan form to your child’s doctor for approval and signature; also the parent/guardian needs to sign the forms and bring the medication to school. The action plan must be updated yearly.

If your child’s medical condition has resolved and no longer requires an action plan, please have your doctor write a note confirming the student does not require an Asthma Action Plan. When the school nurse receives this documentation, it will be recorded in the student’s medical file. At that time, the school will conclude that the medical condition has been resolved and is no longer a problem which must be addressed by the school nurse.

Life Threatening allergic reactions

The Middle Township School District must have a health plan in place for students with a health concern. The health care plan helps to ensure a better understanding of your Child’s healthcare needs, and is reviewed with your child’s teachers and principal. Please take the Anaphylaxis Action Plan to your child’s doctor for approval and signature; also the parent/guardian needs to sign the forms and bring the medication to school. The action plan must be updated yearly.

If your child’s medical condition has resolved and no longer requires an action plan, please have your doctor write a note confirming the student does not require an Anaphylaxis Action Plan. When the school nurse receives this documentation it will be recorded in the student’s medical file. At that time, the school will conclude that the medical condition has been resolved and is no longer a problem which must be addressed by the school nurse.

Diabetes

The Middle Township School District must have a health plan in place for students with a health concern. The health care plan helps to ensure a better understanding of your Child’s healthcare needs, and is reviewed with your child’s teachers and principal. Please take the Diabetic Medical Management Plan to your child’s doctor for approval and signature; also the parent/guardian needs to sign the forms and bring any medication to school. This action plan must be updated yearly.

If your child’s medical condition has resolved and no longer requires an action plan, please have your doctor write a note confirming the student does not require a Diabetic Action Plan. When the school nurse receives this documentation it will be recorded in the student’s medical file. At that time, the school will conclude that the medical condition has been resolved and is no longer a problem which must be addressed by the school nurse.

Seizures

The Middle Township School District must have a health plan in place for students with a health concern. The health care plan helps to ensure a better understanding of your Child’s healthcare needs, and is reviewed with your child’s teachers and principal. Please take the Seizure Action Plan. to your child’s doctor for approval and signature; also the parent/guardian needs to sign the forms and bring any medication to school. This action plan must be updated yearly.

If your child’s medical condition has resolved and no longer requires an action plan, please have your doctor write a note confirming the student does not require a Seizure Action Plan. When the school nurse receives this documentation, it will be recorded in the student’s medical file. At that time, the school will conclude that the medical condition has been resolved and is no longer a problem which must be addressed by the school nurse.

Students Transferring In

All transferring students are required to provide a physical and current immunization record. NJ state law requires a current physical, if the student has not had a physical within the last year, a new physical must be completed within 30 days of entering the school. If your child plans to play a sport, you will also need to have a Preparticipation Physical Evaluation Medical Eligibility form completed. These forms are linked below. We also have copies located in athletic trainer’s office, in nurse’s office, and in front office. If a physical is not provided, within the 30 days, the student will not be allowed to attend school until a physical is provided. A current immunization record must be provided prior to start of the first day.

If it is financially difficult to obtain the necessary immunizations and you do not have insurance coverage, you may contact the County Health Department for an appointment at 609-465-6840. There is a $10.00 fee for the immunizations. Also, you may be eligible for the FamilyCare Program, which offers affordable health coverage for children. Additional information regarding FamilyCare can be obtained by calling 1-800-701-0710. If you are eligible, your child’s physical examination as well as other health-related needs would be covered through this program.

Students Playing Sports

Students who are interested in participating in a sport at Middle Township High School should review the Athletic department website. Information, links and education is provided on the website. Middle Twp uses RSchoolToday as our sport’s platform.

School Health Screenings

The State has provided guidelines as to which screenings must be completed. Screenings will be done by the School Nurse, unless your child has submitted physician documentation or parental exemption of the recommended screenings. All exemptions should be submitted to school nurse by October 1. If transferring into school, during the school year, please notify school nurse within one week of the start of classes. Written referrals will be sent home, as warranted.
