DRIVER’S EDUCATION
BEHIND THE WHEEL
Students who are interested in Behind the Wheel Training MUST:
- Complete the classroom phase of Driver’s Education
- Pass the written test
- Be at least sixteen years of age
- Fill out the green information card completely (IN BLACK OR BLUE INK)
Please include:
- “Full” Name with Middle Initial (no nicknames)
- Sign “Full” Name with Middle Initial on the line with the “X”
- Parent MUST sign the designated line
When completed return everything to the Athletic Department to schedule your 6 hours of behind the wheel.

Turn in to instructor:
- A check for $235.00 made out to Middle Township Board of Education
- Green information card
- $14.00 in cash for permit and decals
- Original Birth Certificate
- Social Security Card
- Written Knowledge Test printed confirmation
- Proof of address (report card, any mail with name and address – magazines, etc.)
If student needs to wear glasses/contacts please have them ready.
